Medical & Immunization Forms
Current Medical forms (Physical and Immunizations) are needed for all students entering Pre K, Kindergarten and 7th grade. All new students to Epiphany (1-8) must also have current medical and immunization forms. The forms must be delivered to the school office by Friday, August 10th, 2018. This is a Florida Law, no one will be admitted to school without these forms. Thank you
Together, we can make a big difference! Keep clipping Box Tops and send them to your child’s homeroom teacher so we can mail them in and get cash for our school.
